†Yes, Multi-engine came first. Commercial and Instructor checkrides normally require a complex aircraft, but by demonstrating complex aircraft proficiency on my AMEL checkride the examiner could waive the complex aircraft requirement on my ASEL ride. Since basically all AMEL are complex aircraft it let me save some money on my Commercial & Instructor ASEL tickets.
‡Yes, Ground Instructor came after Flight Instructor. The training all happened at the same time, but you can't turn the paperwork in to a Designated Examiner - you need to drive yourself to a FSDO. Since this is a pain, I put it off for a year or so. Becoming a Ground Instructor is pretty much redundant if you're already a Flight Instructor, so I wasn't in any hurry.
